Full Time Integrated Case Manager

Position Overview:

  • Works with families to complete on-going assessments, develop case plans, and in the provision of continuing service delivery following intensive family services
  • Participates in initial case planning with the Integrated Family Preservation Services (IFPS) Therapist
  • to establish goals and tasks for continuing service
  • Provides home-based family skills training which may include: Nutrition, Hygiene, Household and Time Management, Home and Child Safety, Basic Parenting, Transportation, Problem Solving and Communication
  • Monitors progress of family using assessment tools and reviews case plan progress with the family
  • on a monthly basis
  • Acts as a liaison with schools to obtain child records, attend conferences, track attendance and support achievement
  • Provides 24 hour, 7 days a week support to staff and remains available during crisis situations

Requirements include:

  • Bachelor's degree in human services is required
  • Requires LAC, LMAC, or LCAC licensure by the Kansas Behavioral Sciences Regulatory Board
  • A minimum of two years relevant experience is required
  • Valid driver's license with a good driving record 
  • Must pass pre-employment background checks and drug screening
