FT Case Manager

Position Overview:

  • Provides home-based family skills training which may include: Nutrition, Hygiene, Household and Time Management, Home and Child Safety, Basic Parenting, Transportation, Problem Solving and Communication
  • Prioritizes identified problems that require resolution, establishes goals and objectives, utilizes available resources and arrives at decisions appropriate to the family 
  • Makes mental health referrals as necessary
  • Assesses each child's safety at every family contact 
  • Remains current on the availability of community resources and assists all families in obtaining assistance
  • Acts as a liaison with schools to obtain child records, attend conferences, track attendance and support achievement 
  • Provides transportation as necessary to meet established program achievement goals
  • Fosters and maintains professional relationships with families through consistent, democratic enforcement of all program policies and rules
  • Provides 24 hour, 7 days a week support to staff and remains available during crisis situations

Requirements include:

  • Bachelor's degree in human services required; Master's degree preferred
  • Licensure by the Behavioral Sciences Regulatory Board as a Licensed Addictions Counselor is preferred
  • Requires a minimum two years relevant experience
  • Valid driver's license with a good driving record 
  • Must pass pre-employment background checks, fingerprinting and drug screening
