Full-Time
Position Overview:
- Assists therapists, case managers and family support workers by linking families with needed community resources and services, coordinating transportation and logistics
- Fosters positive relationships with families
- Provides individual assistance to families to access and utilize community resources
- Assesses and monitors family progress following referral to community resources
- Develops activity plans with families to address barriers such as employment, housing, transportation
- Coordinates group presentations to assist families to address barriers
- Serves as a liaison between families and schools to promote a collaborative process in educational planning for youth, parent/guardians and community
- Serves on school-based and system-wide committees to address educational issues, adjustment problems, safety and program development
Requirements include:
- Bachelor's degree in human services required
- Requires a minimum two years relevant experience
- Valid driver's license with a good driving record
- Must pass pre-employment background and reference checks, drug screen and fingerprint checks
