Family Preservation Services Case Manager - Wichita

Full-Time

Essential Functions:

  • Provides for the safety, stability and well-being of children and families during the referral period 
  • Delivers culturally competent, strengths-based case management services to children and families consistent with the evidence-based practice models
  • Decisions are made on guidelines provided by the supervisor, family preservation standard procedures and policies, evidence-based practice model and DCCCA organizational policies 
  • Meets DCF timelines to connect with the family within 24 hours of referral; facilitate initial family meeting within 48 hours of referral; complete assessment-based case plan within 20 days of referral; and case closure within 3- 6 months of referral 
  • Engages and works in partnership with families to complete assessments and develop case plan goals and objectives. 
  • Meets with the family in the home weekly, monitoring the family's progress towards their case plan goals; and meets monthly with each child individually to assess child's safety
  • Assesses each child's safety with every family contact and reports all suspected child abuse / neglect.  Monitors family progress and service provision using assessment tools, and updates family case plan based on those assessments
  • Provides weekly home-based family skills training which may include: Nutrition, Hygiene, Household and Time Management, Home and Child Safety, Basic Parenting, Transportation, Problem Solving and Communication to assist parents in providing a safe, nurturing environment for children in their care 
  • Works closely with the Family Support Worker to provide services for the family and make decisions about child well-being 
  • Prioritizes identified problems that require resolution, establishes goals and objectives, utilizes available resources, and arrives at decisions appropriate to the family 
  • Completes and analyzes initial and on-going family assessments related to safety, stability and well-being, including mental health, substance abuse, education, emotional, social and physical well-being, parenting and parent-child relationshipsRegular and predictable on-site attendance.

Education, Experience, and Licensure

  • Bachelor's degree in human services, required.
  • Prior work in behavioral health or children and family services, required.
  • Experience working with substance use, preferred.
  • Must possess a valid Driver's License, have in force automobile liability insurance coverage, and maintain a Motor Vehicle Record (MVR) that meets organization's insurability standard as defined by DCCCA policy. Availability of reliable transportation is required for this position.

Knowledge, Skills, and Abilities:

  • Regular and predictable on-site attendance.
  • Proficiency with computers and Microsoft suite of products including Microsoft Word, Outlook, Excel, and PowerPoint.
  • Ability to efficiently operate other office equipment such as telephones, voicemail systems, copy and fax machines, and scanners.

Certain positions will require travel which may include driving personal or company-owned vehicles.  Note: Even with a valid driver's license, certain motor vehicle infractions may render individual's ineligible to drive company-owned or personal vehicles for company business.
