Provides for the safety, stability and well-being of children and families during the referral period
Delivers culturally competent services using a strengths-based approach to children and families consistent with the evidence-based practice models
Meets DCF timelines to connect with the family within 24 hours of referral; facilitate initial family meeting within 48 hours of referral; complete assessment-based case plan within in 20 days of referral; case closure within 3- 6 months of referral
Provides support and resources to family in Nutrition, Hygiene, Household and Time Management, Home and Child Safety, Basic Parenting, Transportation, Problem Solving and Communication
Meets with family in the home weekly to monitor family progress towards their case plan goals and assesses each child's safety
Remains current on the availability of community resources and assists all families in obtaining assistance
Works closely with the Case Manager to provide services for the family and make decisions about child well-being
Documents case activities and information weekly
Requirements include:
A high school diploma or equivalent is required
Requires a minimum two years relevant experience in behavioral health or children/family services;
Valid driver's license with a good driving record
Must pass pre-employment background checks, reference checks, fingerprinting and drug screening
